We are now just about six weeks into the semester, and it seems like a good time to get some feedback from you about how you think things are going. This will benefit me by giving me pointers about what I can improve on in the second half of the semester, and hopefully benefit you by making the class a more pleasant one to take! So please be as thorough and honest as you can be.
There are ten fields to the form, but if your time is short, don't feel compelled to say something for each question; just focus on what you think are the most important things to get across to me.
