Please complete this form if your club/organization has a mid-year officer or advisor change. Only complete the sections for which your group has a change to report. Please do not submit more than 10 officers per club/organization. DO NOT COMPLETE THIS FORM IN APRIL. This form is only for mid-year changes. To complete Group Registration, click here.

Note: Unless you indicate who an officer is replacing, the first two officers on this form will be considered Officers 1 and 2 for your club. Officer 1 must be a Notre Dame student.
